Your home state is entitled to tax your world wide income regardless of where you actually earned it. Any other state is only entitled to tax income that you actually earn in that state.

If you earn less than than the minimum amount required by state law you don't need to file a return although you should do so to receive a refund of any taxes withheld, if any.

Depends on the state.
I am assuming you made money in a different state, but you may still have to file a state tax return in the state you are claiming residency in.

bostonianismo is mostly correct. I have seen states that require a state tax return any time you file a Federal return, even if you are not actually required to file based on your income.
