When we applied for passports for our children; we were told that the signatures of both parents were required. They also asked for a copy of the custody agreement.

at either age 13 or 15 you dont need the fathers signature anymore but you might want to check with your local post office or passport agency

Yes, you must have the fathers signature.

The reason for this is to avoid kidnapping of a child from the other parent.

The only exception is if the father has legally given up all rights to the child through the courts and you have the paper that says so.
