You can get a US Passport at your local post office. Before you go you can get all of the information at either http://www.usps.com/ or http://travel.state.gov/passport/

Fill out the application, bring a picture ID, an official birth certificate, 2 pictures taken especially for passports and $97. Be prepared to wait up to 10 weeks, they're really overloaded with applications due to the change in passport requirements.

Word of advice, they are swamped right now. If you need it in a hurry, pay the extra $60 to expedite it, But you also have to tell them you want it shipped "express mail" We paid the $60, but then they just sent it "priority mail" This is supposed to be 2-3 days. It has been 4 from Seattle to Oregon, and they were to lazy to scan it out, so it's lost in la-la land.
