You complete form P85 (available from your local Tax Office, or downloadable from HMRC website) and send it to your Tax Office with the P45 which your employer will give you when you leave your job.
Of course, if you are not going to be abroad for a complete tax year, no refund will be due at this time - you would have to wait until the end of the tax year in which you leave the UK.

To claim it back you need to complete form P85 'Leaving the United Kingdom' Download it from http://www.hmrc.gov.uk/cnr/p85.pdf and send it in with your P45 leaving certificate and/or your P60 certificate for the year in which you are leaving, and send it in to your local HMRC office.

You can't claim your tax back...it's tax.

You won't get your licence fees, car insurance, road tax, fines or road tolls back when you leave!
