You can get several packages to do this, Trips and Streets from MS, Delorme, etc. All you need is a GPS with either USB or PCMCIA or if your laptop supports is serial connections.
Both Trips and Streets and the Delorme systems are sold with a fairly cheap GPS that will work with them even shows what street you are on. If you already have a GPS and it has USB or some output you can use to connect to your laptop just search for "GPS Mapping Programs" there are even some free ones with GPS interfaces.
