My niece was semi-diagnosed with Middleshmertz due to mid-cycle period pain. Middleshmertz is only supposed to occur at mid-cycle but my niece is experiencing excruciating pain at different times of each month. The pain isn't on a regular cycle of sorts. I feel the doctor just picked a logical choice for her pain and diagnosed her with Middleshmertz not really knowing what's causing the pain. An external ultrasoud didn't show any abnormalities. There was no evidence of Endometriosis. I feel so bad for her. What could be causing her all of this pain?

It sounds like your niece could have endometriosis, but an ultrasound is not capable of diagnosing it. The only way to definitively diagnose it is thru a laparoscopy but before doing any of that she needs to be evaluated by a gynecologist if she hasn't been there yet.

Start with the pediatrician (if a young woman) even before the gyn to see what they recommend for pain control in the mean time.

endometriosis is not diagnosed by ultrasound. she could have ovarian cysts, ruptured cysts or it could have nothing to do with her gynecological region. her pain could be so many other things. if u dont believe the doctor, get a second opinion

Consider an ovarian cyst? PID? a vaginal or transrectal ultrasound may be more diagnostic than an abdominal ultrasound. Seek advice from another gynecologist.
