I would first ask your mother. Most mothers record their child's medical history up to a certain date. Then you can go to your doctor and they will give you a copy of your record. There will be a fee, my doctor charges $35 for a copy. In these records will have all your blood, x-ray and urine tests results also.

Getting copies of your medical records and yes most drs now a day charge for this unless you are sending it to another dr for review.

Another thing that can be helpful is to keep an actual medical journal of your visits to the dr for future reference. I do this myself.

The thing about asking for your mom's medical history and such is also a good one since they will also ask about family history.

you could ask for your last doctors records and go form there or get a :new doctors" new patient form and start filling it out from memory. There are online cmpanies that now offer to keep up to date records secured for your futire reference There are concerns as to privacy but it may be worth considring
