The guy was a Savant, of course. They found the right hemisphere of his brain was nearly, but not quite, dead and the source of the seizures. House deduced that he could lead a more normal life if they cut out the right half of his brain. The down side - he would no longer be able to play the piano.
The father agreed. The operation was performed, and the piano guy lived. The end showed the piano guy could now button his own shirt.
(In the sub-plot, we found didn't really have brain cancer but was just trying to get some experimental happy drugs injected directly into his brain.)

His fingers being messed up was controlled with meds. He had another problem which was that half his brain was dead. House gave his father a choice of removing that half. Which meant he wouldn't be able to play the piano anymore. But he would be able to do other things like button his shirt. His father let them do the surgery. at the end he was buttoning his shirt and smiling.

They removed the right half of his brain because it was practically slowing his processing down. But at the end of the show when he was awake it showed him buttoning his shirt, something he hadn't done since the fourth grade.

Dave Matthews plays a brain damaged pianist who can't button his own buttons but who is an idiot savant. He is concert when he starts to mess up on the keys. When his father gets to him to see what is wrong the man's fingers are curling the wrong way.
Next thing you know all of the interns are in the conference room wondering why they were called in. house is telling them about the case and they are thinking it does not rise to the level of their usual cases. House points out that Cameron and Chase look and smell like they just showered together. He wants them to start running tests to see what is going on with the patient.
Meanwhile in the clinic House is aking his own blood while talking to a girl with a blister on her instep. He has her tie him off so that he can stick himself. She wants to be treated. He tells her she has a blister and looks very thin in her belemic blue lipped way. Foreman catches House passing the blood off to a nurse and tells Cameron and Chase.
Getting back to the patient, Dave starts having seizures and so they are trying to figure out what is going wrong. They bring in a piano to test his eye hand coordination. He finishes off a piece House had been writing in high school with out even thinking.
Cameron has found plane tickets to boston to go along with the mysterious blood drawn and wants to know if House is leaving. She gets Chase to go with her to break in Houses condo and see what they can find and what they find is that House has been calling Massachusetts General Hospital. When she gets back she gives this information to Cuddy.
Cuddy calls Mass General to find out why they are trying to steal House. They say they are not. She says then he is either going there because he is sick or because he has a friend. They say his doctors are not his friends.
Cuddy goes to Wilson to find out what the doctors do for a specialty and he tells her brain cancer.
Wilson gets House aside in his office and asks him why if he has cancer he did not tell him. House blew it off as something he would rather do away from work. Wilson is thinking theyhave nothing that special in Mass general right now.
The Patient is getting worse so they decide to drill holes in his head and do a catscan inside his skull to see what is frong with him as the can't pinpoint a problem.
Meanwhile Cameron is thinking she is going to have to find a new job because House is leaving and Wilson tells her she has to keep it to quiet but House has brain cancer.
Next thing you know all three of the interns are giving him the look and asking to see his test results. He is reminding them they have a patient. They are giving him the look, the you have cancer look.
As for the test results they show that there is nothing vascularly wrong with Dave's brain but the right half of his brain is infact dead.
House is saying mostly dead as he takes yet another keyboard to the patient and has him play something proving the right side of his brain is still functional, meaning that the man must be suffering from some sort of auto immune disease. He has foreman run tests to see what disease it is.
Back in his office Foreman is trying to tell him he likes him when house tells him it is just the cancer thing.
Next Cameron is coming with her letter of recommendation for House to sign. He does saying how glowing his comments must be. And then she come as though to hug him and plants one on him instead only he kisses her back and as he does she pulls a blood vile assembly to try and stick him but he catches her and tells her she can find all the info she wants under the name Luke and Laura down in the lab. She points out to her that he kissed her back. He points out to her that he didn't want her leaving never having known what it was like.
Down in the Lab House's tests look like he has less than a year to live, and the tumor is inoperable, so the crew goes crazy trying to figure out what can be done. They start calling around to find better programs. There are but House is not interested. So, they keep testing.
Back in his office Chase is giving House a hug as well saying he has to.
Meanwhile, Dave is beginning to get better but house is having an idea. He goes over to Cuddy's house. Once again he wakes her up in her satin robe. He tells her he wants to do a hemispherectomy on the piano player. She tells him he needs the father's permission. She also tells him she is there for him with the cancer. He gives her a hug and Grabs her but, saying "One small grab for man one giant thrill for mankind." She starts to head to the bedroom and he starts to follow. She tells him to call the Make a Wish Foundation.
At the Hospital Chase is finishing a sample for Cameron to look at and they all notice something it there that shouldn't be.
In Matthew's room House is telling his father that he wants to cut out half his brain, the dead half to restore much of his thinking ability. He will no longer be able to play the piano but her will be able to be more independent. Relusctantly the father agrees. House sells it to him as the man is not happy but he could learn to be.
The operate on the man and it is a success. He can not speak yet but he can already button his own pajamas which he could not do before. The father is pleased.
The staff catches up to house and tells him that he does not have cancer,. He has syphillis. Cameron is suddenly glad they never had sex. He wants to know if they told Boston and they had. He is upset because those were not his results. They belonged to another man in the hospital. House was using those records to get into a study that involved putting a permanent pain blocker right into his skull. Suddenly they realized he was just trying to score another fix. Now, Foreman says he does not like him any more.
Back in House's Office Wilson is lecturing House on Cancer patients and how they wishe they had friends to ease their suffering. House has many who stood by him and he treated them as though they were nothing not telling them the truth. he recommends House make some amends.
On his way from the Hospital House find his team talking to one another in a restaurant and instead of merely walking on by, this time he takes a chance and walks on in. EOE
