Depends if they invite you in to sit down, or if there isn't a rug in front of the door. But, the proper attire is to take you shoes off, and on the other hand, maybe not if your looking at making a quick escape for some off the wall reason. Totally depends on you!!! If they ask if you could remove your shoes, do so. If not, your choice.

I never wear shoes at all, regardless of whether I'm going to visit friends or strangers, regardless of whether it's a formal or informal situation. I haven't worn footwear for ten years and don't intend to put any on ever again, no matter if I'm going to the beach, a party, a wedding or a funeral. Bare feet are so much better, more comfortable, healthier, stronger, more respectful to Mother Earth.

Yes to both - tracking dirt from outside thru the house on your shoes just makes for more cleaning. I don't do it at my home so wouldn't do it at someones elses.

I look at it like this, if I was in Japan, yes...that is part of their cultural beliefs...If you live in America, screw THAT! If I have to take my shoes off to enter someone's home, I won't be going to tea...
