To give an honest answer, I would have to say her use of sexuality. Look back during the earlier 80s when she first came out and was known for her provocative activities that she displayed in concerts; I think she redefined and brought out the use of sexuality in music - where as it was taboo (I think) - and tried to make it of an interest to society. That's my answer.

She pays smart people to\
1. Write her music for her.
2. Get concerts for her.
3. Budget her money very carefully.
4. Keep her in the gym, the dance studio, etc. to keep her in shape so she can keep entertaining.
5. Constantly thinking up controversal things to sing about, to do, of which all the catholic churches go up in arms over, and THAT creates plublicity.
This woman is very smart.

Good management.
She also has a curiosity for other cultures, which sets her far apart from the Britneys and the Christinas, etc. She takes non-trendy things and makes them mainstream before any other popular artist does.
