No, you don't have to have a parent/guardian signature...but I strongly advise you NOT to use the tanning bed. I have heard horror stories of girls getting awful rashes from the beds not being sanitized. Also, tanning WILL lead to premature aging, wrinkles and skin cancer. If you want a little color to your skin try a sunless tanning lotion!

No matter what your age, please avoid tanning beds. I know a golden tan is pleasing to the eye, but go for a good spray tan or a tinted lotion. You really are borrowing trouble to use tanning beds. Not only will you cause your skin to age prematurely, but I've heard some stories about serious skin rashes that are hard to treat. Try not to become addicted to the whole "tanning" thing. It's not that fashionalbly correct anymore, and you will look younger years longer if you avoid the beds and take the sun in small doses using good, good protection. Moderation is key.

Yes you have to be 18 list medications your on as this could result in serious burns even over the counter med's.

nope... i've used one since I was 16 without anything signed. You just have to sign a "I know I can get burnt and this can cause skin cancer" type of waiver.
