The code for Egypt 002 or +2 then for Cairo 02 or change the code of the city according to the city you are calling. If you are calling a mobile (cell phone) it'll start with 012 or 010 instead of the city code.
For the charge it depends on your cell phone line and your country. You didn't mention where are you calling from. Most countries charge you local cell phone call in case you used a phone card. To be sure you should check with your company or ask about it again in the section of your country to get the right answers.

I've used my friend's cell phone to call to Egypt, using a phone card. I doubt it will charge you, and it is actually cheaper to use a phone card, than dialing directly from a cell phone, because that will be VERY expensive, i've learned from experience.
on your cell phone bill, the number you dialed to egypt won't show up, meaning your friend who lives in Cairo, their phone number won't show up on the bill,
but the 1-800 number you dialed for the phone card will.

First dial "00" then the country code, then the city or area code if any than the number. The double zero is for international. You can use the long distance operator but it will cost you more.

ur cell phone wont be charged if ur calling from a phone card. only the minutes used on ur cell phone will apply.

you have to dial 002 02 this for cairo
cell phone 002 012 .. .... or 002 010 ......
ofcourse it will charge the phone card and the cell itself as it is a local call in your country

Your phone card should be an 800 number, so you'll be charged like you were calling an 800 number in the US (i.e. it may apply to your minutes, that's it).
EDIT: I've used my cell phone to call other countries with phone cards, the calls were treated like calls to any other 800 number. No international phone call charges ever showed up on my cell phone bill.
