2 Impermeable layers of rock with a layer of permeable aquifer sandwiched in between. The water is generally under pressure. It might find it's own way out or wait for some one to tap it with a pipe.

You have an impermeable layer on top of an aquifer, where the water is under sufficient pressure. If a vent is created through the impermeable layer (by earth movement or drilling), the water comes to the surface.
