LOTS. They frequently use body doubles, so you can be fooled that you actually saw someone nude, when it was not them. Like what's-her-name in the Wedding Crashers, Vicne Vaughn's chick. Wasn't her!

vivaca refused to get nude for a movie so halle berry got the role (not that she was fully nude but she showed some skin)

Racquel Welch was always famous for refusing to do any nude shots...wouldn't even do topless stuff.
Always seemed strange to me, because she sure did pose for a lot of awfully low-cut, heavy cleavage photos in real titillating (love that word) poses.
