My shoulder was jammed in a car accident 4 years ago and even now, I sometimes get pains in my shoulder and arm - sometimes it feels like an ache, sometimes shooting pains, sometimes just tingling. Is this normal, 4 years later? How do you distinguish it from heart attack symptoms?

Oh yes, they can hurt on and off for a long while! I had a horse riding accident when I was a teen and landed heavily on my left shoulder. Now at the age of 46 it still gives me a twinge once in a while!
The pain from a heart attack is very severe and sustained (not shooting). You feel as if a tight band is around your chest and breathing can become difficult. There may also be pains down the left arm and the jaw can also hurt.

It means the shoulder was permanently marked. As for the heart attack, heart attack's pain is like if "someone holds your heart with the fist" and then it goes to the jaw or shoulder. They last at least 20 min.

Heart attack is NOT stabbing pain that lasts few seconds, pain that is caused by the use of a joint (but effort can trigger the pain) or pain that worsens when breathing deeply.

Any pain is not normal. You should have it checked out.

all the time , I had my left bicep ,surgically re-attached in may 2002 , and it still hurts all the time , .
