I'm not sure what exactly you're asking, but if you're thinking of sending somebody a "legal" looking letter to collect money owed, think about it. Don't misrepresent yourself as a lawyer or collection agency.

Send the person a letter saying the money is overdue and this will be the last notice before sending the account to a collection agency, which will affect their credit rating for any and all future loans, credit cards, mortgages, etc...
I would also mention that any and all collection agency fees, court fees and lawyer costs would be added on to their bill, if you did send it to an agency.
Send the letter via registered mail, so they have to sign for it and you have proof that they received it. It only cost about 2 dollars.
If you call a collection agency they might have some info for you as well. Tell them you will contact them if the account is not settled.

A certified letter has no format. Certified is simply a method of mailing a letter that provides proof of delivery. Simply write your letter, address it as normal, and take it to the post office. Tell them you want to send it Certified, Return Receipt Requested. Pay the postage and it's done.
