How about the gift of YOU?? Not knowing your situation but if she's near give her a card that says something to the effect of you will spend time with, help around the house, go to a movie with, go shopping with, something or anything that allows just the two of you to be alone and get to know each other. Not just one day but say once a month for the entire year.Take this time in life while you have it. Now, if Granny is far away that will be tricky. Gifts like magazine subscription to something she is interested in (gardening, home, cooking??) can last all year. Granny's usually have pretty much everything they need but would love to have your love all the time. Just a thought.

IF she doesn't have a current photo of you, it might be nice to head down to Walmart and have a picture taken--or have a friend take a nice photo and give it to her in a nice, new picture frame.
Offering to help her take care of things around her home (cleaning, sorting through old mail, etc.) for a few hours would probably thrill her.
Flowers always make grandmothers feel special.

Older people are hard to shop for because most of what they need or like, they have already accumulated. But one thing that often gets overlooked is an extra-special set of writing paper or notecards. Older people often don't feel engaged with email culture and like to send a personal note. (I do myself, and I'm only 38). Add a pretty pen for a personal touch. If she's up to writing a note, I promise you she will appreciate these.

A pin or broach. A good size watch or pendant watch. A pair of slippers. A Dr Scholls Foot Bath.

What does she like to do? Does she have a hobby or something that she likes to do in her spare time?
Take her somewhere special, spend the day with her.
Take her out to eat.
Does she like to feed the birds/squirrels? Get her some bird food or something.
Get her a nice plant/flower that she would enjoy.
