Put any experience you have had, including CPR/First Aid/Babysitting classes. Don't mention any past history of child abuse. Put your age, your availability, whether you have your own transportation. Put your pay requirements or 'pay negotiable.'

I would state the radius of where you would babysit (maybe within 10 miles from your home).
I would give the time and days you are available.
References, if asked.
Any experience.

do put if you have taken any special classes or have a certificate in First Aid/CPR.....don't put the history of insanity in your family....
