Is it just a matter of what location they chose and how many hours they want to work? Can a doctor's office nurse go work in an ER if they wanted to or is it a different certification?

If you have your nursing license you can work at any facility. The difference is wheither the doctor is a specialist or what department in the hospital you want to get work in. A nurse can have their RN certification and license and work most anywhere, but if you wanted to work in the cardio department, it would help greatly if you had extra education or experience in that field. Some specialites do require certification, it all depends on what you want to do.
