Elvis' nickname given to him by the press, was "Elvis the Pelvis". The press was all about rhyming back then. Within the Memphis Mafia, his nickname was "Crazy." I'm not sure if Elvis gave himself that name or the mafia did.
The song was Hound Dog and the clip they showed was from Milton Berle.

As far as I know, the only song that Elvis sings in Forrest Gump is "Hound Dog"
