I think it depends on where you work. If you are a professional it is very common. Sometimes I do sometimes I don't. Usually I say you can just ask for me by my first name. If you are one of several with the same first name, you can always just use the initial of your last name.

Legally, you are entitled to your right to privacy.
Depending on what line of work you are in, you should exercise the correct measure of caution for your own safety.
For instance, if you work for a collection agency, I would probably use a fictitious first name.
But, if you work as a sales person why not give your last name if it is requested, unless of course the individual just doesn't seem right.
It's a judgment call, life is full of them.

As a good customer service gesture: Yes.... and your boss can fire you for poor customer service performance.

You should probably ask your supervisor. I would say no, but there may be certain jobs where you'd need to. Check with your supervisor.
