No, a healthy person might experience some dizziness and a reddened face from the blood rushing to their head, but it is extremely unlikely that they would die.
Someone with a poor heart or clogged circulation, or suffering from extreme blood pressure issues (high or low) might be in greater danger.

Usually yes, but why waste the time when a bullet in the head works quite well, or if that's not practical for some reason, a hard bashing with a hatchet is eminently satisfactory.

Most likely, yes. All the blood would rush to his head and just stay there for 3 hours, he's a dead man.
