The Massachusetts state income tax witholding form is Form M-4, similar in function to the federal W-4.

Ask your employer for a form W-4. This is the form that is used to change your number of dependents or any additional amount of money you want withheld. He is required to provide this form to you. If there is any problem you can always get a W-4 form from www.irs.gov. Fill it out, it is very easy, and give it to your employer.
