The correct answer is most of the above. Petroleum (oil) is made of hydrocarbons. Hydrocarbons are not all formed from plants and animals -- it rains liquid methane (natural gas) on the Saturn's moon Titan and nobody calls that "fossil" fuel so hydrocarbons can be natural minerals thus A is a correct answer. But decaying plant and animal remains can also form methane so B is also correct. I'm not sure about C but D is half right because methane and other petroleum components are gaseous at standard temperature and pressure.

The answer they want is B. But the real answer is the Sun, because the Sun allowed the plants to grow and/or the animals to grow, and subsequently die, be buried and turn into fuel. However, some theory say that fossil fuel is not really from organic matter, but I am sure your school is not so advanced as to consider anything but standard rhetoric (research the Internet for other theory).

B. Plants and animals under high temperature and pressure gets fossilised and get converted into carbon and hydrocarbons. The process takes thousands to millions of years.
