look at their eyes, see if they have a gassy look, listen to the way they speak if they are slurring their words or having any difficulty speaking

Every one of your respondents has assumed that your question refers to alcohol. If you do not drink you die of thirst. If you only drink alcohol you will die of some other disease.
Now, assuming that you were refering to alcohol, you have to be aware that there are substances other than alcohol that will cause people to behave in an abnormal manner.
Breath small is a good indicator, speed of response to normal stimuli would be another indicator. Absense of awareness to reality would be another.
A longer term method would be to collect reponses to questions posed when under influence compared with the responses to those same questions posed at a more sober time. Memory is a poor friend of the enebriated.
But without a doubt!! Breath smell can be the only one.
If you have so little trust in someone and for personal reasons this is a big issue, go find someone else that you can trust.

The easiest way is to have them exhale through a breatherlizer. These gadgets can be bought on ebay for under $100.
Some diabetic patients smell like a drunk at times without going close to any alcohol and other sign or symtoms of intoxications can be traced to other drugs.
The fool prove way is the blood test, but that is not so practical and portable in most circumstances.

Only a blood alcohol test would tell you someone had been drinking "without a doubt". Anything else would leave room for doubt because they would not "prove" it.

Well, I know if I've been drinking when I wake up next to a chick who seemed to be good looking the night before. But not so much the next day.

Very simple-- smell their breath. Alcohol will be excreted thru skin, saliva, and mucous membranes for a long after a person has been drinking heavily.

Do the same things the cops do; ask them to walk a line, or touch their nose, etc.
