My grandmother is Puertorican and generally, when she uses the term she is reffering to my grandfather( who is white) and she uses it in a negative connotation, I believe the way she uses it and how it has evolved (from what she told me) it's something like 'white man' but pejoratively 'dirty white man'.

A white man. And it's not really a racial slur. It's a slang for white person. There's a female and masculine form too, gringo for man and gringa for female.

Usually it means a white person (female form is gringa), but sometimes is applied to any non-Hispanic.
It is slang but not necessarily pejorative. Depends on the tone of voice.

Many years ago looked this word up in a Spanish-English dictionary. From what I can recall, the dictionary said something to the effect of "(esp. mex.) Foreigner or stranger.", referring to non-mexicans.
