It can be a cafe for nurses.
You can make the whole cafe look like a hospital. The tables and booths can resemble hospital beds. The waiters and waitress can have the uniforms.
In the bathrooms have the soap pumps be that a syringe. So when you pump the soap it is like giving an injection only soap squirts out. Also in hospitals the sinks are operated by pedals. Toilet seat tops could resemble a bed pan.
Door hands could be shaped like stethoscopes. Clipboards could be used to take orders.
You could have IV drips for drinks.
Also, there could be plaques of the great accomplishments of nurses, such as nursing theorist and things.

Makes no difference if it's in a big city or a small town. I'd call it Grandma's House. I'd use the decor from way back when. Use your imagination, as to what your Grandma's house was like. Serve the food on the table as your Grandma did, in bowls, and the meat on a platter, gravy in a gravy boat. Make it seem like they were really eating at Grandma's house. I used to have a restuarant like this in my area. It was great. Have the restuarant divided up into small rooms. This way they feell like they're really at home. For a large croud you could have a big room with several tables. This was my favorite place to eat.

Theme "Beautiful foods from everywhere and everywhen."
Good or fake antiques and exotic items. Furniture and decore, warm and friendly but just a little "off the edge."(like "fix" a broken chair with a piece of pipe or a crutch.)

The Cop Shop - have police style deco and offer police, firemen, other emergency workers and nurses free meals. Or "The Station House" with firefighting gear.
More ambitious? "Ramses" and serve North African style dishes in an Ancient Egyptian decor.
