The best thing he can do is to present himself to the hospital and they will tell him what to do.
No Dr today will take patients without insurance.
Some communities have hospitals that specifically take patients with no insurance so check with your local hospitals.
Merely make a call,don't give names etc just call your local hospitals and ask them if they take patients without insurance.
If in asking you fail to find one,you might ask one of the hospitals what they would recommend what alternatives you have.
In the meanwhile,keep the hand immobile and iced to prevent swelling.
Should you notice fingers turning blue or if the fingers start getting cold when you haven't been using the ice,you should just present yourself at an emergency room and go from there.

Go to a local hospital emergency room, they cannot refuse to treat him and will X-ray his hand and cast it. They will also probably write him a prescription for pain killers, which will make him more comfortable. He can work out with the hospital a payment plan. A publicly funded hospital cannot, by law, refuse treatment to anyone, no matter what they ability to pay is.

Well, try to just go to the nearest Emergency room. I live in NJ and I went to the Emergency room for a sprained hand. Because I don't have insurace, and I dont make much money, I applied for Charity Care through the hospital, and I got it. That means, I'm a charity case, they have to help me for free. Altough you do wait in long lines, and have your own doctor, it's good if you're in a jam like yourself and your brother.

How about the emergency room of your local hospital? Or a teaching hospital? Somewhere that can either do a sliding scale, or do hardship application for. Take care.
