If the service is good then yes... you should make up the extra to meet 20%. When gratuity is on the bill it is for large parties usually and is only 15% of the bill BEFORE tax. It is a sign of class to tip 20% for good service.

In Europe, where the gratuity is routinely added to the bill, I will leave small additional amount if the service was good.
But in the U.S., the gratuity is generally only added to groups of 6 or more. And even then the waiter usually has the option of adding the sure 15% or taking a chance on what the customer may voluntarily leave. I figure if he adds the 15%, then he has made his choice...for both of us. No additional tip unless I'm drunk or just overwhelmed by the quality of the service.

There are certain situations where I would include an extra gratuity
If the waiter/waitress's performance was beyond what would be expected I would add a cash tip, due to the fact that the gratuity is included in the bill it is shared between the wait staff, bus person and cooks as well as the dishwashers.

I definitely would if it was worth it. The gratuity is added in for a reason in restaurants. I feel that the server should get extra when it is deserved, if the service was good.
Gratuity is usually added in with larger parties, which is more work. I work as a server, and it is a lot of work for big parties, and sometimes for tips it is just not worth it, so the extra can really help out.

You surely dont have to. If you pocket book is overflowing then go ahead and make someones day, they are probably working on minimun wage plus tips. However if the service was the pitts it would tend to anger one that knows they have already gotten a tip that they didnt deserve.

No... and we only tip if the service warrants it, so if we go to some place and the service is included in the bill already then we find somewhere else to eat....prices are generally expensive enough and we refuse to tip for bad service...
