(Am in Canada; my tables look like that. Would be interested to hear from Canadians whose tables don't, though.)

Where does the fork go in the States?

The fork goes to the left of the plate, tines up. If salad forks, oyster forks, etc. are used, they go to the left of the main course fork.

The knife goes to the right of the plate.
The spoon goes to the right of the knife.

The fork belongs to the left of the plate tines up. If your first course is salad, then the salad fork and then to the right of it, the dinner fork. Then the knife to the immediate right and then the spoon. A dessert fork would be at the top.

Our tables are set like that too. Fork on the left, knife beside the plate on the right side with the spoon beside the knife. Glass above the knife and spoon.

In the U.S. a basic table setting is the fork on the left, the knife and spoon on the right.

It gets more complicated if you are attending a real formal affair because then you have forks, knives and spoons for everything.

The fork goes on the left, the knife on the right, next to the plate, with the teaspoon next to it. Desert spoon can be at the top of the plate.

it is very simple:

forks go on the right side, from biggest to smallest.
knives go on the other side, from biggest to smallest, and after that goes the spoons next to the knives, in biggest to smallest order.

the other way is to arrange them in order they will be used, the closer in they are to the plate, the more recent the use.

ex: meat course, salad course, soup, dessert, (big fork and steak knive, then little fork and knife, then spoon, then utensils for dessert.
