Mr. and Mrs John Jones request the pleasure of your company at the marriage of their daughter Melissa Mary Jones to Mr. John Jacob Stevens, son of Mr. and Mrs. Gregory J. Stevens on Saturday the twenty first of June, Two Thousand and Seven at 5:00 o'clock in the afternoon at Christ the Redeemer Church 2009 State Street, Cleveland, Ohio
Of course this will be layed out on the invitation very nicely, not like I typed it...

On another card you should have
Reception at
The Fox Hollow Inn
Jericho Turnpike
Woodbury, Ohio

Cocktail Hour at 6:30 PM
Dinner immediately following

Then another card to be returned to you with a stamped envelope
Mr. and Mrs. .............................

will attend...............will not attend.......................

When you go for your invitations, you will see that the people that will help you will show you many different wordings. You indicate to them that you want both parent's names included.
Good Luck to you.

typically the beginning says something like

Mr. and Mrs. insert your parent's names here request your presence at the marriage of our daughter NAME HERE. She will be united in marriage to Groom's Name, son of Mr. and Mrs. ........

Have you been to a book store? I would go to Barnes and Noble or something like that. There are SO many books there that can help you with the wording. Good luck.
