What is the ratio of students to teachers?

What is their percentage of students who move up every year (and what factors do they use to decide - you don't want a school who just moves everyone up, even if they can't do the work!)

What extracurricular programs are available - early drop off or late pick up?

Are you an accredited school?
What curriculum do you use?
What do you do when you see a child excelling or a child falling behind?
What are your discipline practices?
What is expected of my child?
What is expected of me?
Do you have an open door policy?
Can I show up and visit the school unexpected? If not why?
Are your teachers at least minimally qualified? What are your minimal qualifications?
Is there a PTA/PTO I can join?
What are your rules on my child's safety?

Here are a few I hope this helps. I'm sure you will think of more when the time comes.

Have you tried greatschools.com? They will tell you how each school scored on a variety of items. When I moved I checked out my son's school before hand. They will also, if you choose to, send you emails about things you can do with and for your child to help her. They have different emails for different grades.

I would ask how many students there are per teacher. What is there reading program like? What do they expect of you and your child? Things like that.

I'd want to know if there was a special emphasis on arts, music, or sports. Are there resources for gifted kids as well as kids with learning disabilities? How often are the kids changing classes? Are there extracurricular activities available?
