No. Don't fill it out. It would come directly from Oprah.com and you would've had to enter to win. I dont think Oprah has had any sweepstakes lately, either.

It is a SCAM! Ask yourself, Did I buy a ticket? Do I have a Ticket? Did I fill out a Ticket form? The answer is No, so it is a scam.

All of Oprahs things are done on her show. if you were to win that amount I am sure that you would be the featured guest on her show.
