I dont want my employer to know this and the form asks both if Ive had mental illness and what meds Im on among other things like if Ive had ulcers or cancer.Its pretty in depth.What should I do ?I take my Meds and nobody knows but my family.I really want this job but it is management and people can be cutthroat.Nobody can know this about me.

Whatever you put on there is confidential. Your employer can not share this information with anyone else. I had to do this and I have borderline personality disorder. If you already have the job, they can't fire you because of this. It's considered discrimination. I know it's scary, but not putting it down can cause problems with coverage and your employer. If the insurance is through your work, you can not be denied coverage due to pre-existing issues. So I think you should deffinately go ahead and put it down.

Wrack_n_ruin is totally right about this. No employer can ask any question about your health in the USA except to ask: "do you have any condition that could prevent you from doing the job?" and the only acceptable answer to that question is yes or no.

Perhaps the form you're presented with isn't actually from your employer, but is a form about your employer-sponsored health insurance? If that is the case, insurance companies know that they can't disclose any of that information to the employer.

If you are applying for an increase in coverage, or the plan isn't a "group" plan, then these questions are valid and permissible.

I don't know where you live, but in the U.S., this sort of questioning is totally illegal. Consult with your state's Department of Labor, or Equal Opportunity Employment office.
