Practically speaking, disrupting the sensory systems of the CNS will most affect the individual's cogition -- this kind of damage would be most evident in lesions of the frontal and parietal cortices, as well as in the limbs of the internal capsule. There are a broad number of ways in which this may occur, most common of which is vascular infarct (i.e. stroke).

You really have to narrow it down a bit. Where is the lesion? It could be anywhere in the brain, in the brainstem, or along the spinal cord. Lesions in the brain would cause for cognitive losses. Lesions in the brainstem may cause loss of sensory modalities or paralysis. Lesions in the spinal cord could also cause loss of sensory modalities and paraliysis, but the extent of the loss would depend on the level of the lesion.
