As said above it's not the same expression but in french it's "fille d'anniversaire" and in italian " Ragazza da compleanno", but if u mean the girl that pops out of the giant cake, there's no such expression in italian.

"Birthday girl"? Not sure there is a french word for that, word by word we can say " la fille anniversaire" but it doesn't mean anything in french.
