I know that this is going to sound like what everyone will say, but you should give him a VERY romantic evening with you as dessert.
I know that may sound a little blunt, but if you do it right, not wild or anything, he will love you even more.
My wife and I have been married for 7 years, and this weekend is our anniversary. And a romantic evening is exactly what I have been dreaming about all week. When all we do is "Work! Work! Work!" we really need a day when we can really be together. Just share each other and nobody else. Just push everything aside for a few hours and pretend the rest of the world doesn't exist.

So 4th Anniversary is fruit and flowers. Go with romantic. Get an elegant honeymoon suite hotel room. Get him a card ahead of time that says "so our 4th anniversary it is traditional to get your spouse fruit and or flowers come see my interpretation." Pack an overnight bag even if you work the next day. get him in the car you drive and take him to your special room. Then have a huge bowl of strawberries marked "fruit" sprinkle the bed with rose petals and maybe have a nice table with a light meal and a small vase of red roses that are marked "flowers" and turn on the romance! The modern interpretation is "appliances" see what "appliances" you can add to your overnight bag to make it extra special!

Here's something creative that will be appreciated and you will have fun doing:

Make up a crossword puzzle and have all the hints be personal inside moments and jokes that only you two would know about. Put it on a poster board and add some clipart and some pictures to it!

The crossword puzzle could be time consuming and a bit frustrating ... you could do the same idea with a WORDSEARCH or WORDFIND puzzle.

Put your computer skills to work! Use Excel, get clipart from Yahoo Images Search, copy them, and reformat them, use a gluestick to put on posterboard.

Use graph paper, and pictures from magazines if you are not as good on the computer, and it will give it more of a homemade feeling.

As far as lists go:
Fourth anniversary (traditionally) is fruit/flowers. The new (modern) gift is appliances though.
