If you only worked for 3 weeks, you most likely dont meet the minimum requirements to file. However, if you're not required to file, and you had federal and/or state taxes withheld, you would want to file so that you could get these taxes refunded.

If they paid you over $600 total, they had to report it to the IRS. (Anything less than that no one is required to report.)

So, if they reported it to the IRS and you didn't, that could make you a prime candidate for an audit. Those are about as much fun as a good root canal (without anesthesia.) A lot of people think that they only audit people who make lots of money, but that's simply not true. They audit randomly and they audit those with red flags (like leaving off income reported by the employer.) And even if you don't end up owing them much, it would still be a pretty lousy experience.

Report it (assuming it goes over the $600 threshold, of course.) Do the right thing and you won't have to be looking over your shoulder. (By the way, they can come back years later to do the audit.)

If that was your only income for the year, it's probably not enough to have to file unless it was from self-employment - then the limit is $400.

Doesn't matter if the employer notified the IRS! As mentioned, anything under $400 you don't have to worry about.

I was told that you only have to file if it was above 500 dollars. I've never filed under that and had a problem.
