Can someone explain to me what causes the characteristic 'punched-out lesions' in gouty arthritis?

Just wanna know how the punched-out lesion happens and what is it caused by.

My understanding is that this is caused by deposits of urea. Mine break through the skin as my body rids itself of the deposits. Hope this helps.
