My daughter had one on her neck, and when I asked our pediatrician about it she said that what she does is to tie a knot of dental floss around it, super tight, and then go to bed.

That's what she did with the one on my daughter's neck, but with a piece of suture material. It just fell off later that day and wasn't painful at ll.
