I think a kiss on the cheek is fine on the first date, but I would normally wait until at least the second or third date to kiss on the lips. It's up to the girl, you have to try to figure out whether she's ready for it or not.
At the end of the night when you are saying good night to her, if she kinda stands around and loiters and looks up at you, she wants a kiss.

There's no special time to wait. You just go with the flow, if your feeling each other, you kiss. Usually when a chick tries hard to prolong something like a simple kiss even after your both digging each other, she'd hiding the fact that shes slutty usually.

I'd say it depends on your age for one thing, if you're 12 then take lots of time, if you're an adult, just go w what feels right, whether that's the first date or the fifth. Check out her body language, if she wants it, you'll know. Can't schedule these things, just relax and go w the flow.

You're SUPPOSED to wait till the third date before kissing the lips.... everyone i know breaks this rule

there is no exact time..you'll just feel it..but that feeling may be in the good and in the not so good side..think first..maybe you're just feeling the "heat"..that's a different story compared to the feeling of longing for a kiss of someone you really love..think not just twice..but as many times as possible..don't rush!
