Cardiologists are internists first. They subspecialize to deal with diseases of the heart.

Cardiothoracic surgeons are general surgeons first. They subspecialize to restrict their surgical practice to hearts and lungs (mostly).

Cardiothoracic surgery is a surgical subspecialty. Cardiology is a medical subspecialty. The former will happily take a saw to your breastbone and fillet you open. The latter will put a catheter in your groin and run it up into your heart but otherwise pretty well stay non-invasive.
