Don't use heat besides very hot showers or baths
I used to have sholder pains all the time from swimming, and eventually it got so bad I went to a doctor. Nothing serious, but anyway, he told me many things to do to help it. If it's sore a lot, see a doctor like me. My physician saved me, and now I'm not sore anymore.
If it's a one time thing, i reccomend you get two huge plastic bags and fill them with ice. Lay down somewhere like a couch and put them over your arms, the back and front sides(you may need 4 bags). Stay like this for at least 10 mintues. Watch TV or something. Although it gets very cold, it helps tremendously.
Keep using painkillers, ibeprofen is best, if you need to

Much pain is from muscles below is an example of what may help (based on headaches).
Begin with a couple swigs of molasses or a couple of bananas (natural muscle relaxers) daily - magnesium (which regulates many things in the body) and potassium (a needed building block for muscles).
Drink at least 1/2 gallons of water per day. Running a body low on water is like running a car low on oil is the analogy the head of neurology at UCDavis told my husband about 10 years ago.
Now to the cause - muscles - your back, neck shoulders and head have tender spots. They are knots in the fibers of the muscles called trigger points. It makes the muscles tight which makes them press on nerves and other things causing the pain.
The cure - start with a professional massage, (if this does not do it you probably need a chiropractor as well) you will also want to go back over any place you can get to 6-12 times per session up to 6 times per day rubbing (or lightly scratching on your head) every where that is tender until the knots go away. The place where the skull connects to the spine press up under the edge of the skull (to get to those muscles).

I had a bad shoulder, massage machine helped, but till it worked I found that if I bent my arm up on to the pillow for support that was the most comfortable position.
Now for the great bit, try self Hypnosis, it works, its a bit like counting sheep.
Start counting back from 500 at a slow measured pace after tell your self to relaxe and get a nice deep relaxing sleep, 500 499 498 etc do not worry when you lose your place just carry on from where you like.

i really do feel for i am going through similar had to visit g.p. last week due to no sleep and taking half an hour to turn over when in bed due to the pain!! g.p. said sprained my shoulder and as pain killers or ibrofen was not working now on co-codamol for the pain (second prescription) hope you get some painrelief too and a good nights sleep soon

Use anti-inflammatory, also use therapy like warm and cold shower, alternate between cold and warm shower every few minutes in the shower, do some yoga or relaxation exercise then take heat pack in bed with you and listen to soothing music before you go to bed. This will help you relax your muscles as well as mind and you will sleep better.

urrm i would say muscle rub but i dont know whether you will go out for it at this hour sooo... get a spoon of brandy burn the alcohol off, put some hot water in a cup with some honey and then put the brandy go to bed lay down you should sleep...
