Contact Labor & Industries for your state, they should have all information regarding worker's compensation records. Do you not have any receipts/records of what went on back then? I know I still have mine, there's too much important info there to toss it. Did you have an attorney, he will have proof of the settlement, also.

With a claim that old it will prove difficult, if not impossible. Businesses are only required to keep records for so long.

First try calling the company you were working for at the time. They may keep the records. If not they may be able to at least tell you who their Worker's Compensation carrier was in 1990, and perhaps they will have a record.

Another option would be to try contacting the doctor(s) who treated you for the injury. Perhaps they have the insurance company and claim number in your file, which will make it much easier to track down the information.

That's it. I'm all out of ideas.

Good luck!
