No. Just follow the regular rules of grammar. Capitalize the start of the sentence and proper names.

"Amen" is capitalized only when at the beginning of a sentence. I guess it could be in reference to the TV show which had Sherman Hemsley (of "The Jeffersons" fame) in it. Otherwise, no. "Amen" is not capitalized.

No. Amen is just the Hebrew word for 'it is so'.

By the way, it is correctly pronounced with a broad 'a' sound, like in 'armour'.

Usually, it is a sentence all its own, since it means something like "so be it." If a comma is placed before it, then using a small "a" is okay.
