I would stare at first. But then I would wonder how good of a doctor he is. If he really is a good doctor, then it would make the doctor trip much easier to talk to the doctor. Because you know how uncomfortable it is to talk to serious doctors as a teenager. Some things to talk about are awkward.

Well, my church's old preacher had a mohawk right after he was first hired, so I daresay if a [very contemporary] Church of Christ preacher [in his 20's] could have a mohawk, so could a doctor. You see, though, he had decided to shave his head, so he went with a mohawk for about a while, then got married, and THEN shaved the rest of the mohawk off. :-P He was awesome.
I would love a doctor with a mohawk.

Anyone who says "it doesn't matter" is fooling themselves. A doctor who came in to your exam room with a crazy-looking mohawk would certainly face a credibility issue. Doctors need to project an image of professionalism and trust to their patients, and a mohawk would not help that.

I had a classmate in Medical School who had tatoos covering every inch of his body that was covered when wearing pants and long sleeves... arms, legs, torso, back, stopping only at his neckline and wrists. I lost touch with him after graduation, but I always wonder what sort of problems he has as a doctor if his patients were ever to see that.

He was a good guy and a good student, but let's face it, looks and appearance DO matter to at least some degree.

Well if he was a good doc I wouldn't care but I might ask him what the Mohawk stands for etc.

Hey if he's a doctor and good at it what does it matter how his hair is styled. I wouldn't mind.

I would be more concerned with his/her affiliations and licenses and which board is he/she a member of and also his/her experience as a physician.
