Sit in a quiet room with the lights off. Close your eyes. You know you have studied well and are prepared. Visualize yourself in the exam room taking the exam and knowing the answers. Imagine yourself confident that you got an excellent grade. Try to visualize everything including your feelings/emotions, what you are wearing, what you had eaten, everything! After you've done that several times, you will feel calmer and more prepared!

During her high school years my daughter had the same problem and we successfully solved it. Naturally she had to overcome fear, which was the primary obstacle. The way she managed, finally, to accomplish this was to take test after test after test. Every opportunity for testing which came along without a charge she enrolled and took the test. That was testing for state jobs, school tests, scholarship exams, everything. It worked for her (by the time she was a junior she was successful on the PSAT and scored with the nation's top students). By "learning" to take exams she overcame her phobia. Maybe it will work for you.
