You will probably need to show 1 or 2 forms of idenity since you don't have your original license. Check with you local DMV office, they will tell you what you need to do.

Well that depends what country ur from.. In my country u need to go
and apply for a duplicate license in the nearest RTO.

we go to the revenue office that also does a dmv servie, give them ur id information, date of birth, ssn , and then they'll look u up in their system, charge u a fee, take ur new pic then give it to u. costs about 25 bucks here

Go into whoever issues them, report yours lost, give them what they need to see/know, pay whatever fees are involved, and get your duplicate.
If you are in the US, all they need is your name, date of birth and social security number to find it.
