I think the best way to learn grammar is to read, watch movies and talk with people. You begin to internalize the rules, and learn to speak better. You can read grammar books, but it's hard to apply them in a conversation. Practice, practice, practice.

Vocabulary is a different matter. You can get calendars with "a word a day" that teach you a new word each day. Readers' Digest has a vocabulary quiz. When you read, keep a dictionary next to you. When you find a word you don't know, first guess what it is from the context. Then look it up in the dictionary. You may be surprised! Do this for *every* unfamiliar word. You may forget the first or second time you see the word, but by the third time, you'll start to really know it.
