All depressive disorders are Axis I.

Major Depressive Disorder:
- Mild: 296.21 (296.31 if recurrent)
- Moderate: 296.22 (296.32 if recurrent)
- Severe: 296.23 (296.33 if recurrent)
- Severe with Psychotic Features: 296.24 (296.34 if recurrent)
- In partial remission: 296.25 (296.35 if recurrent)
- In full remission: 296.26 (296.36 if recurrent)
Dysthymic Disorder (mild, chronic depressed mood): 300.4
Depressive Disorder NOS (not otherwise specified): 311

(The post above me is wrong. Axis I includes mental disorders. Axis II is personality disorders and mental retardation [not expected to change], Axis III is medical condition and illnesses, Axis IV is psychosocial stressors and environmental factors and Axis V is a rating scale from 1-100 that estimates how well a person is able to function daily.)

The above post is correct-depression is always axis I. The only addition is 309.0 Adjustment Disorder with depressed mood and 311 Depressive Disorder NOS (Not Otherwise Specified). These codes are the same in ICD-9 as well as DSM IV and nowadays insurances are requiring ICD-9 codes. Only a few differ from those listed in DSM IV.

Its axis 1, not sure what you mean by serial num, if you mean how its coded for insurance or for type like NOS, PTSD?
